Aviation occurrence reporting, from the first report to closure
In 50 words
Occurrence reporting is how an operator captures safety events, from small deviations to accidents, so each one is assessed and acted on. In eAviora every report moves through eleven stages, from triage and classification to risk, investigation, actions and closure, and each stage states what it still waits on.

What it does
Four things, done on the record
Report from anywhere
On the web, on eAviora Field with or without signal, or through your public reporting page.
Identified, confidential or anonymous
For a confidential or anonymous report, the reporter’s contact details are never written to the occurrence.
Classified, then scored
Each occurrence is mapped to a risk family and scored on the ICAO 5 by 5 matrix, with a written rationale.
Ready for ECCAIRS
Export an occurrence as an E5X file, coded from its classification, or as a PDF summary.

In the product
An occurrence, under investigation
- The stage it has reached, of eleven, and the next step it waits on.
- Its initial risk, placed on the 5 by 5 matrix.
- The people on the record, with the safety owner assigned at triage.
The rule it holds
No risk score without its reason.
An occurrence cannot leave initial risk until someone writes why they chose that severity and probability.

Short answers
Can people report confidentially?
Yes. A reporter chooses identified, confidential or anonymous. For confidential and anonymous reports, contact details are stripped before the occurrence is written.
Can we send occurrences to ECCAIRS?
Yes. An occurrence exports as an E5X file, with its ECCAIRS codes mapped from the classification, and the export says when a mandatory attribute is still missing.
Can crews report from the ramp without signal?
Yes. eAviora Field keeps the report on the device and sends it when the signal is back.
